Instrument Checkout

Instrument checkout begins on the first day of class for each semester. Submissions prior to the first day of class will be processed after the first day of class.

By submitting this form, you indicate that you have read, understand, and agree to the terms outlined herein:

  • Instruments remain the sole property of 麻豆社. Students may not sell, transfer, pawn, or otherwise give up control of the instrument while it is assigned to them.
  • The instrument will be returned in the same condition it was received at the time of check-out, less normal wear. Students agree to assume liability for all damages done to the instrument while in their possession.
  • The student is responsible for the fair market value of the instrument, including all parts and accessories, if it is lost, stolen, damaged, or destroyed.
  • Students are to keep the university-owned instrument in the assigned locker.
  • Students agree beverages and/or food will not be kept in the locker.
  • The instrument must be returned prior to graduation, between semesters, and upon demand, unless prior approval has been arranged with the student’s studio instructor and the Department of Music Head. Summer instrument check-out must be approved by the Music Department Head.
  • Final grades may be withheld for unreturned or damaged instruments.

You will be notified via email when your request has been approved.

  • Submissions with email addresses other than @valdosta.edu will be discarded.
